Study: UK Breast Screening Criteria Miss Most Under-50 Women Who Develop Cancer

Cambridge and ICR researchers say a Boadicea-based risk assessment would flag 26.5% of under-50 women for review and catch 34.8% of those who develop breast cancer within 10 years — versus 1.4% and 4.4% under current NICE rules.

Current NHS breast screening criteria miss most younger women who go on to develop breast cancer, according to a new study by researchers at Cambridge University and the Institute of Cancer Research.

The Study

The team used a risk calculator called Boadicea, which combines family history, lifestyle, reproductive history, and genetic information into a risk score, the BBC reported on August 4. If all women under 50 were assessed this way, the researchers estimate about a quarter — 26.5% — would be categorized as above-average risk and referred for further assessment. That group would include 34.8% of women under 50 who develop breast cancer within 10 years.

The Comparison

The current NICE criteria would refer just 1.4% of women under 50 for further assessment — catching only 4.4% of those who develop breast cancer within a decade. The main reason for the disparity: 73% of women under 50 who develop breast cancer within 10 years have no family history of the disease, and family history is the key criterion in the NICE guidelines.

"This is a leading cause of death in women under 50 in the UK, and most of those cases have no family history at all," the researchers said.

The Trade-Offs

Lead researcher Dr Juliet Usher-Smith said the NHS should review its criteria in light of the findings, while acknowledging that referring more women would add to workloads and could cause some women unnecessary checks and anxiety. Dr Sowmiya Moorthie of Cancer Research UK, which funded the work, said "any changes must be accessible and equitable, taking into consideration the anxiety that extra referrals might cause for women." NICE welcomed the findings and acknowledged the potential of multifactorial risk tools, but said current evidence does not warrant a change to its guidelines.

The Context

About one in seven women in the UK develops breast cancer in her lifetime, though only 5-10% of cases are linked to inherited genes. Most cases — 96% — occur in women over 40, and 25% in women over 75, but breast cancer remains a leading cause of death in women under 50. The debate is not just about numbers: it is about how to balance earlier detection against the anxiety, cost, and capacity pressures of screening a much larger population.
